What is Case Management and Targeted Care Coordination (TCC)?

Case Management is a program to help people receiving Medicaid access and manage all the community services they or their children need. Case Managers make sure everyone involved in providing treatment is on the same page and that each client is happy with their services.

Case Managers coordinate treatment plans for people struggling in more than one area of their lives and dealing with a mental health diagnosis. They assess their clients’ overall mental states, help them set long-term goals, and connect them with the resources they need to successfully manage their conditions on their own. Case Managers meet with their clients regularly — often weekly — to monitor their well-being, address their questions or concerns, and connect them with resources.

TCC is for children participating in Youth Empowerment Services (YES) and is similar to Case Management, but includes a person-centered plan allowing for additional services.

 

What can Case Managers and Targeted Care Coordinators help with?

Case Managers and Targeted Care Coordinators assist in finding and scheduling needed services such as:

  • Speech therapy

  • Medication management

  • Medicaid transportation

  • School/IEP

  • Vocational rehabilitation

  • Day treatment

  • All other Medicaid services

  • All other community resources

  • Primary care physician

  • Specialists

  • CBRS

  • Counseling

  • Neurological testing

  • Psychological testing

  • Home health

  • Housing

  • Occupational therapy

In addition to helping find and schedule services, Case Managers and Targeted Care Coordinators can help in a number of ways, such as:

  • Assist in utilizing natural supports in each community

  • Advocate in complex systems such as school and healthcare

  • Ensure satisfaction with services received

  • Assist to find resources in times of crisis

If you have been denied beneficial services such as CBRS due to requirements set by Optum, a Case Manager can keep you on track in order to help you begin those services again.
